Submit a Class Action Claim Form - Instructions

If you want to participate in this Settlement, you must complete and submit a Claim Form electronically or mail in the Claim Form postmarked by March 20, 2017. If you are a Class Member and you complete and submit a Claim Form, then you may receive a payment if the Court approves the Settlement and it becomes effective.

How to File Online

After Clicking the button "Start Your Claim", you will be asked to login using the "Tracking Number" and "Unique Identifier" included on the Claim Form mailed to you. If you did not receive a Claim Form or have lost yours, please contact the Settlement Administrator. Please read the Class Notice before you complete the Claim Form
